CLANDESTINE LAB PROPERTY CHECK AND SUMMARY
To be completed and submitted by members of the public seeking to know if a property has been identified by York Regional Police as having been used as a Clandestine Laboratory, including a Marihuana Grow Operation.

Requests must be mailed to York Regional Police Headquarters, or delivered, in person, to the Freedom of Information Office located at 17250 Yonge Street in the Town of Newmarket.

CONSENT FORM FOR POLICE RECORD CHECK FOR NON-PARENT CUSTODY APPLICANTS
Form to be used by individuals applying for custody of a child (or children) to allow York Regional Police to search the National Repository of Criminal Records.

POLICE INFORMATION CHECK (PIC)
A PIC is intended for applicants who are seeking volunteer or employment positions with agencies requiring a PCRC, along with a record of local police involvement.

POLICE CRIMINAL RECORDS CHECK (PCRC)
The PCRC is a search of the RCMP databases. A criminal background check is usually required for employment purposes or by-law and immigration puposes. 

COTTAGE WATCH
The York Regional Police Marine Unit have initiated a Cottage Watch Program for all seasonal residents in the Lake Simcoe patrol area within York Region. In an attempt to reduce crimes against property, the York Regional Police will conduct periodic spot checks of properties in the area and notify you in case of an emergency. FINGERPRINT AND PHOTOGRAPH DESTRUCTION FORM

MUNICIPAL FREEDOM OF INFORMATION AND PROTECTION OF PRIVACY ACT FORM
Persons seeking to make a Freedom of Information request must complete and submit this form.

POLICE VULNERABLE SECTOR CHECK FORM
Form to be used by individual applying to work or volunteer in a position where they will be in direct contact with children or members of the vulnerable sector. The applicant must submit the form in person at either location indicated on the form. 

PROJECT LIFESAVER PARTICIPANT AND CAREGIVER AGREEMENT
YRP has a program which uses electronic signaling devices to aid in the search for, and rescue of, lost persons who suffer from one form or another of diminished mental capacity or ability to communicate.

SUSPECTED COUNTERFEIT BANK NOTES AND FORGED PAYMENT CARDS
Use this form when a passer is not present. Complete without delay and forward with counterfeit funds.

VALUABLE PROPERTY RECORD
Log your valuable items on this form. If the items are ever lost or stolen in future, the log will be helpful to police.

VEHICLE COLLISION SELF-REPORTING FORM
A collision which has occured in Richmond Hill, Markham or Vaughan may be reported at the centre providing it does not involve a) danger to on-scene motorists, b) personal injury, c) any Criminal activity, such as impaired driving or stolen vehicles, d) a government vehicle of any kind, e) vehicle which is transporting dangerous goods, f) damage to third-party property, such as a parked car where the owner is not at the scene, g) damage to private, municipal or highway property, h) pedestrian or cyclist or i) an uncooperative driver. Use this form to report your motor vehicle collion and attend the Community Resource Centre.

VICTIM INFORMATION PAMPHLET
This pamphlet provides information to assist with understanding the investigation process. In addition, a list of telephone numbers of various supporting agencies has been provided.

VULNERABLE PERSON FLAG RECORD REGISTRY APPLICATION AND AUTHORIZATION
Information is collected by means of an application and authorization form. The collected information is entered onto the York Regional Police computer system, which is accessible by every police officer. In the event a vulnerable person requires police assistance, valuable caregiver contact and vulnerable person descriptive information can be accessed by police.  Please return the completed and signed form to any police district or mail the completed form to: York Regional Police, Records Branch, 17250 Yonge Street, Newmarket, ON, L3Y 4W5 or send it via email to vpr@yrp.ca.
