Remote Care Monitoring
Remote Care Monitoring (RCM) is a partnership program between York Regional Police and Toronto Grace Health Centre (TGHC).
This program provides eligible participants with a GPS Locating Pendant that assists officers in locating persons who have gone missing, are lost or wandering. TGHC operates a call centre that contacts and coordinates a response with police to help locate and return participants to a place of safety.
How Remote Care Monitoring Works
Remote Care Monitoring helps individuals remain safe and independent at home and in the community.
Referral to the program
Referrals to Remote Care Monitoring (RCM) are submitted by members of York Regional Police Community Engagement and Wellbeing based on recent involvement with police and emergency services.
Participant receives a GPS Locating Pendant
Eligible participants are provided with a GPS Locating Pendant connected to a 24/7 monitoring service operated by Toronto Grace Health Centre (TGHC).
Monitoring and response
Using virtual care technology, TGHC works alongside community partners to support medical and non-medical needs and responds when alerts are received.
Call centre coordination
When an alert is triggered, the call centre may:
- Contact caregivers or support persons
- Coordinate an appropriate response
- Escalate to police when required
Police support when needed
If York Regional Police is contacted, the call centre can provide live location updates and direction of travel information to assist officers in locating the participant and helping return them to a place of safety.
Eligibility
Eligibility is determined by Community Engagement and Wellbeing.
Additional eligibility criteria include:
- 60 years of age or older
- Valid OHIP coverage
- Living alone or with a caregiver in stable housing
- Having a primary contact person
Please note: Due to the cellular and magnetic features of the GPS Locating Pendant, individuals with cardiac implantation devices (for example, pacemakers) are not eligible.
