About Remote Care Monitoring
Remote Care Monitoring (RCM) is a partnership program between York Regional Police and Toronto Grace Health Centre (TGHC). This program provides a GPS Locating Pendant to eligible participants that assists officers in locating persons who have gone missing, are lost, or wandering. TGHC has a call centre that will contact and coordinate a response with police to help locate and return the participant to a place of safety.
How does Remote Care Monitoring work?
RCM provides services that help individuals remain safe and independent at home and in the community. Using technological advancements in virtual care, TGHC works in conjunction with various community partners to address medical and non-medical needs. Referrals to RCM are submitted on behalf of participants by YRP members of the Community Engagement and Wellbeing bureau based on recent involvement with police and emergency services.
Eligible participants will be provided with a GPS Locating Pendant that links to a call centre run by TGHC. TGHC provides 24/7 monitoring that responds to alerts from the Pendant which may include contacting caregivers or escalating to police when required. When YRP is contacted, the call centre provides live updates about the person’s location and direction of travel to assist officers in locating them.
